A Modified Virus Injected Into 16 Patients Just Reprogrammed Their Immune Systems From the Inside

Sixteen people with multiple sclerosis and related autoimmune conditions saw their symptoms improve after receiving a single injection designed to rebuild their immune system from within, according to results published this week in the New England Journal of Medicine. The approach skips a step that's normally central to this kind of treatment entirely.

Conventional CAR-T therapy, already used to treat blood cancers, involves removing a patient's T cells, genetically modifying them in a lab to recognize a specific target, then infusing them back into the body, an expensive, multi-week process. 

This trial instead used a modified lentivirus, engineered by the biotech company Shenzhen Genocury Biotech, to deliver those same genetic instructions directly inside the patient's bloodstream, turning their own T cells into disease-fighting cells without ever removing anything from the body.

The mechanism worked as intended. The reprogrammed T cells multiplied over time and depleted the B cells and autoantibodies responsible for attacking healthy tissue in the first place, patients with multiple sclerosis specifically showed improved motor and cognitive function along with reduced fatigue, while those with muscle-affecting autoimmune conditions showed better strength scores and less inflammation. 

Crucially, the B cells that eventually repopulated the patients' systems didn't produce the same harmful autoantibodies, suggesting a genuine immune reset rather than a temporary suppression.

Outside experts called the results a real step forward while urging caution about reading too much into a small trial. "This is a very exciting proof-of-concept study," said David Simon, a clinician-researcher at Charité, University Medicine Berlin, noting that in-body CAR-T production is both cheaper and faster than the lab-based version. 

Dai-Shi Tian, one of the trial's lead investigators at Huazhong University of Science and Technology in Wuhan, struck a similarly measured tone: "The responses are promising signals, but they are not yet definitive evidence of efficacy or permanent restoration of immune tolerance." Larger trials are the next step before this approach could move anywhere near routine treatment.
